At Crusader Claims RCM LLC, we operate a Revenue Cycle Management (RCM) service that assists healthcare providers with medical billing, claims processing, and related financial services. As a business associate under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we process Protected Health Information (PHI) on behalf of our healthcare provider clients (covered entities). This Privacy Policy describes our practices regarding the collection, use, and disclosure of information through our website (www.crusader.claims) and in connection with our services.   • We may de-identify PHI for internal analytics or aggregate it for benchmarking, in compliance with HIPAA.

We do not use or disclose PHI for marketing, sale of data, or any purpose not authorized by our Business Associate Agreements (BAAs) with clients.

  • With service providers (e.g., hosting, analytics) under confidentiality agreements.

  • As required by law, or to protect our rights/safety.

  • Only as directed by our healthcare provider clients or permitted/required under HIPAA and our BAAs.

  • With subcontractors (sub-business associates) who sign BAAs and assist in services.

  • To payers/insurers for claims processing.

  • In de-identified or aggregate form where no individual can be identified.

We do not sell PHI or share it for unrelated purposes.

 

4. HIPAA Compliance and Security:

 

We maintain administrative, technical, and physical safeguards to protect PHI in accordance with HIPAA Security Rule standards, including encryption, access controls, audit logs, and employee training. In the event of a breach involving PHI, we will notify affected clients as required by HIPAA and assist with their notification obligations. Our clients (healthcare providers) are responsible for providing patients with their own HIPAA Notice of Privacy Practices.
